A medical certificate (sometimes referred to as a doctor's certificate) is a statement from a physician or other health care provider that attests to the result of a medical examination of a patient. It can serve as a "sick note" (documentation that an employee is unfit for work) or evidence of a health condition.

Medical Opinion Forms
Medical opinion forms should be filled out by acceptable medical sources such as your treating doctor. Such forms can save time for your doctor and your Social Security disability attorney. The information from these forms will help the Social Security Administration determine if your disability meets a listing in the Listing of Impairments and to determine your residual functional capacity.

Sometimes, your doctor will not have all the information needed to fill out a Social Security disability medical form, but it can be helpful for your attorney to know what your doctor does not know in addition to what he or she does know. It is better for your doctor to indicate that he or she does not know the answer to a question than to leave it blank.

Usually your attorney will ask your doctor to fill out the form early in the case. If your condition improves later, you’ll still have the form to demonstrate that you were disabled for a certain period. If you change doctors, your attorney will probably want your new doctor to fill out a second medical disability form. Sometimes it’s better to have your doctor fill out a form later in the case, and sometimes you won’t need to use a form at all. Your disability lawyer will advise you on the best course of action for your specific case.
